Request Your Medical Records
For patients and/or legal guardians, we offer two convenient ways to request medical records for your personal use or to send a copy of your records to a Healthcare Provider (Continuing Care).
Option 1: Request Online (Fastest & Secure)
Request and receive copies of your medical records securely through our online portal. This process provides:
A tracking number to monitor your request status
A faster turnaround time by avoiding mail delays
A quick and easy process—takes only a few minutes
How to Submit an Online Request:
- Choose your type of request
- Records for yourself: Click the "Submit: Request My Records" button
- Sending Medical Records to a healthcare provider: Click the "Submit: Continuing Care Request" button
- Obtain medical records from another provider: Click the "Request: From My Provider" button
- Follow the prompts to enter your information.
- Upload a copy of your government-issued photo ID or Healthcare Authority Paperwork to verify your identity.
- Submit your request and receive a tracking number for updates.
Please note: all medical imaging requests for personal use are done through PocketHealth. You will receive instructions on how to access imaging.
Option 2: Submit a Paper Request Form
If you prefer, you can complete a Medical Records Release Form and submit it via email, fax, mail, or in person.
How to Submit a Paper Request:
- Choose your type of request and download the specific form
- Complete the appropriate form and have the patient or their legal representative sign the form
- Send the completed form using one of the following methods:

Third Party Requesters
If you are a Third-Party Requester (such as an attorney, insurance company, or disability agency) seeking medical records for a patient, you can submit your request online using our secure portal or fax/email your request.

How to Submit an Third Party Online Request:
- Click the "Submit: Third Party Request" button
- Fill out the required requestor details and patient information.
- Upload a copy of the patient’s signed Authorization for Release of Medical Records
- The authorization must be signed by the patient or their legal representative to proceed.
- Ensure the form is legible, valid, and includes all necessary details
- Submit your request and receive a confirmation number to track its status.
Option 2: Submit a Request via fax or email
If you prefer, you can fax or email a request with an authorization form signed by the patient or their legal representative.
How to Submit a Paper Request:
- Submit your request with an Authorization for Release of Medical Records that is signed by the patient or their legal representative
- Send the completed forms using one of the following methods:

Important Notes:
- Incomplete or unsigned authorizations cannot be processed
- The cost for records will be billed to the Third Party Requester in accordance with state-regulated pricing

Our Team uses industry leading technology and processes allowing us to fulfill these Medical Record requests in 5-7 business days. These requests can be done on a computer, tablet or smartphone and you will receive a confirmation via the method you select (Text or Email) when the records are delivered. This request portal is designed for patients or parties with the power to make healthcare decisions on behalf of a patient. Parties with the power to make decisions on behalf of a patient will need to provide documentation of your authority to make healthcare decisions for the patient to be able to use this portal. Third parties must send their requests to the medical facility directly.
