Request for Medical Records

Request Your Medical Records

For patients and/or legal guardians, we offer two convenient ways to request medical records for your personal use.

Option 1: Request Online (Fastest & Secure)

Request and receive copies of your medical records securely through our online portal. This process provides:
✔ A tracking number to monitor your request status
✔ A faster turnaround time by avoiding mail delays
✔ A quick and easy process—takes only a few minutes

How to Submit an Online Request:

  1. Click the "Submit a Patient Personal Request" button below
  2. Follow the prompts to enter your information.
  3. Upload a copy of your government-issued photo ID or Healthcare Authority Paperwork to verify your identity.
  4. Submit your request and receive a tracking number for updates.

Option 2: Submit a Paper Request Form

If you prefer, you can complete a Medical Records Release Form and submit it via email or fax.

How to Submit a Paper Request:

  1. Download the Patient Personal Release Form
  2. Complete the appropriate form and have the patient or their legal representative sign the form
  3. Send the completed form using one of the following methods:

Third Party Requesters

If you are a Third-Party Requester (such as an attorney, insurance company, or disability agency) seeking medical records for a patient, you can submit your request using our secure fax or BHS email.

How to Submit an Third Party Request:

  1. Submit your request with an Authorization for Release of Medical Records that is signed by the patient and their legal representative.
  2. Send the completed forms using one of the following methods:

Important Notes:

  • Incomplete or unsigned authorizations cannot be processed
  • The cost for records will be billed to the Third Party Requester in accordance with state-regulated pricing

Our Team uses industry leading technology and processes allowing us to fulfill these Medical Record requests in 15 business days. These Patient Personal requests can be done on a computer, tablet or smartphone and you will receive a confirmation via the method you select (Text or Email) when the request is received. This request portal is designed for patients or parties with the power to make healthcare decisions on behalf of a patient. Parties with the power to make decisions on behalf of a patient will need to provide documentation of your authority to make healthcare decisions for the patient to be able to use this portal. Third parties must send their requests via fax or email.
