Request and receive copies of your medical records securely through our online portal. This process provides:
A tracking number to monitor your request status
A faster way of receiving records versus picking them up at the medical facility
A quick and easy process—takes only a few minutes
How to Submit an Online Request:
- Choose your type of request
- Records for yourself: Click the "Submit: Request My Records" button
- Sending Texas Retina medical records to an outside healthcare provider: Click the "Submit: Continuing Care Request" button
- Follow the prompts to enter your information
- Upload a copy of your government-issued photo ID or Healthcare Authority Paperwork (required for legal guardians) to verify your identity
- Submit your request and receive a tracking number for updates
Our Team uses industry leading technology and processes allowing us to fulfill these Medical Record requests in a timely manner. These patient requests can be done on a computer, tablet or smartphone and you will receive a confirmation via the method you select (text or email) when the request is received. This request portal is designed for patients or parties with the power to make healthcare decisions on behalf of a patient. Parties with the power to make decisions on behalf of a patient will need to provide documentation of your authority to make healthcare decisions for the patient to be able to use this portal.
